Community health support
This is provided from our Community Learning Disabilities Teams.  They are based in different parts of the city working together to make sure that the health needs of people with a learning disability are met. We use a person centred approach and will listen to you and the people who know you best. We’ll then base your care on your health needs. To find out more about how we can help you, download the booklet about our team at the bottom of the page.
